Here's a fun one, that was brought to my attention recently.
In Index: Chaos, Tzaangors had the option to take Instruments of Chaos, which provided +1 to advance and charge rolls.
In Codex: Thousand Sons, Tzaangors instead have the option to take a Brayhorn, which provides +1 to advance and charge rolls.
Due to GW's decision to change the name of the upgrade, and their own Index flowchart, Tzaangors can take both a Brayhorn and an Instrument of Chaos, and get +2 to advance and charge rolls. Not bad!

Now that I've had a chance to think about this a little more (and have already fielded one "is this going to be allowed in tournies" question) I'd have to say that this would not be allowed in tournaments locally that I run. (of course play however you want outside of my tournies)
My reasoning behind this is you must use the current datasheet for your unit, which gives the updated version of the equipment - which you also have to use in it's most current form. Them renaming it and going with the logic that it's a different upgrade is stretching things a little thin... it's their musician upgrade for the unit. The rules exploit it allows would be a stretch to say that's the intent.
If anything this is more of a case in the other thread that indexes have served their usefulness and it's time to stop using them for matched play. I was certainly leaning that way myself already... and it will probably come to pass sooner than later.

Some quirks of RAW (Cypher calling in Assassins) are cute, and mostly harmless. This would actually be pretty broken, and so I'm glad it's being put to rest now.

I really like the index system when it came to bridging the gap between rule sets. But I also like the attitude that they have had their day, the age of index is over, the time of the codex has come!
